阿法替尼是一种口服药物,是表皮生长因子受体(EGFR)和人表皮受体2(HER2)酪氨酸激酶的不可逆抑制剂。Ⅱb/Ⅲ期随机研究表明,与安慰剂+最佳支持治疗相比,阿法替尼+最佳支持治疗治疗既往一二线化疗及EGFR-TKI治疗失败的非小细胞肺癌患者,未改善患者总生存,但显著改善了无进展生存期,提高了ORR和8周疾病控制率。另一项研究(LUX-LUNG2)结果显示,伴有常见EGFR突变类型的大多数(61%)患者接受阿法替尼治疗后,肿瘤显著缩小、肿瘤进展时间较晚、生存期较长。这些结果初步证实阿法替尼是比较有前途的靶向药物。
阿法替尼为不可逆转的ErbB家族阻断剂,能抑压信息传导和阻隔与癌细胞生长和分裂相关的主要通道。由于通过ErbB家族信息传导机制可由多个同二聚体跟异二聚体引发,所以同时抑制多个ErbB家族成员(如 EGFR, HER2, ErbB3 及 ErbB4),能较有效地中断下游信息传导。
